NEW PATIENTS
We are currently ACCEPTING NEW PATIENTS and have SAME DAY APPOINTMENTS as well as SATURDAY APPOINTMENTS.

If you want to become a new patient, please call our office on 480-598-4145 to schedule a new patient appointment, a few questions will be asked over the phone so we can schedule an appoinment for you.

Initial visits with your doctor usually take more time than follow up visits, Please come in 30 minutes before your scheduled appointment to allow us enough time to check you in before your appointment.

If you have access to a printer, please print and fill these forms before coming in to reduce your wait time.
* New Patient Registration Form, can be faxed to 480-598-4346
* Release of Medical Records, DO NOT SIGN this form till you are in the office

When coming in for your appoinment you need to bring the following with you,
* Drivers License or Photo ID
* Insurance card or a proof of Insurance if you have insurance
* Copay if your Insurance Plan requires
* If you are currently taking any medications, please bring them with you or a list of all medications, you can fax the list to 480-598-4346 before your visit.


If you will not be able to make your appointment please call us at least 24 hours so we can open up the slot for another patient.
